Applications with special identifier requirements
There may be exceptional situations where the sponsor or host of a Wellness Checkpoint® implementation has specifically elected one of the exceptional identifier options as required to meet the demands of their host application. While these are unlikely to apply to your application, they are listed here for full disclosure.
- Where the Wellness Checkpoint has been configured to require an additional identifier
The sponsor or host of this implementation may configure the system to require you to provide your patient, employee or member number. You will be asked for this information only once, at the time of your first assessment. This additional identifier
cannot be used to access your data within Wellness Checkpoint, but it can be used by the sponsor or host to integrate your assessment data with other data. If it has chosen to require this additional identification, the sponsor or host has assumed responsibility for maintaining the privacy of your data outside the Wellness Checkpoint®.
- Where the application has been configured to require preassigned access codes
The sponsor or host of this implementation has chosen to provide you a preassigned personal ID and password. By doing so, they have assumed responsibility for maintaining the privacy of your preassigned codes. You can exercise an added level of protection by changing the original password supplied in your certificate to a password of your own choosing.
- Where the application has been configured to accept ID's and passwords from a host application
The sponsor or host application of this implementation has assumed all responsibility for login and access control external to Wellness Checkpoint®. Please consult the privacy policy statement of the host application.
